I am 41 year, now suffering from vein thrombosis, almost cured. Can I undergo surrogative treatment? What is the expense? What about legal fomalities?
Dear Ms.Bindhu,

It is not clear whether you want to be a surrogate or you are looking for a surrogate mother. Considering your medical condition and tendency for DVT, you are not advised to be a candidate for surrogacy. However, if you are looking for a surrogate mother, you will have to meet a doctor in order to get yourself and your husband evaluated. This will help to decide on the course of treatment and legal formalities.

I have irregular menstrual cycles, and my doctor told me that I have polycystic ovary syndrome. What does this mean?
Dear Ms.Geetha,

Polycystic ovary syndrome (PCOS) is a common hormonal disorder that is found among women of reproductive age. The exact cause of PCOS is not yet unknown and in certain cases, may be genetic. In this condition, the ovaries produce an excess amount of androgens or male-type hormones that prevent the normal maturation of the egg. Very often, the ovaries tend to have a multicystic appearance on ultrasound due to this arrest in maturation. Higher androgen production could also lead to excess hair growth or hirsutism on the face, chest, and abdomen. Women with PCOS often experience difficulty in conceiving owing to fewer normal ovulations. Besides, most patients with PCOS are resistant to insulin and hence, must be screened for diabetes. As ovulation tends to be infrequent due to PCOS, the uterine lining does not shed regularly or, in other words, menstruation does not occur regularly. This could result in an increased risk for developing endometrial cancer.

However, you don't have to worry if you have this condition. You would be prescribed medication to regulate the menstrual cycle, to help you ovulate and to reduce excessive hair growth. If medications fail to create the desired outcome, an outpatient surgery known as laparoscopic ovarian drilling would be recommended.

How long should a couple try to conceive before going to a specialist or a fertility expert?
Dear Ms.Fathima,

If you're below 35 and have had frequent (about two or three times a week) unprotected sex for at least a year without conceiving, then you should consult a fertility expert. If you are a woman above 35 years of age, then it is a good idea to see a doctor earlier, perhaps after around six months of trying. Your doctor will offer you advice and perform some initial assessments. If required, you might be referred to a reproductive endocrinologist.
